Grommet in the eardrum

A grommet in the eardrum is a treatment offered when there is persistent negative pressure or fluid in an ear. It is most often children who have grommets inserted, but adults can also benefit from a grommet in the eardrum.

What is a grommet in the eardrum?

A grommet in the eardrum is a small spool of plastic or rubber with a hole through it.  The grommet is designed to be inserted into the eardrum and has a flange on each side so that it can stay in place by itself. The hole in the middle of the grommet ensures that equal pressure is created on each side of the eardrum, and that any fluid in the middle ear can drain out into the ear canal. 
 
A grommet will normally remain in the eardrum for 3–6 months and is then expected to be gradually extruded. The grommet loosens from the eardrum and falls out into the ear canal, while the eardrum behind it closes again.  
In many cases, a single grommet treatment is sufficient. In certain cases, the grommet treatment must be repeated after some time. 

Reasons for having a grommet in the eardrum

• Persistent fluid in the middle ear behind the eardrum
• Frequent middle ear infections
• Persistent troublesome negative pressure in the middle ear
• Persistent hearing loss, earache or dizziness as a result of the above 

Examination

You will be seen by a consultant and have an ear, nose and throat examination through a microscopic examination of your ear. In addition, you will have a hearing test and a pressure measurement of the ear. Based on your medical history and the results of the examination, the consultant will advise you on which treatment is best for you. In some cases, a grommet in the eardrum will be the best solution to relieve your symptoms.

Treatment

The grommet can be inserted into the eardrum under local anaesthesia – this is done on an outpatient basis. 
 
If a grommet is to be inserted into the eardrum of a child, general anaesthesia can be offered. 
 
You will normally be able to resume your activities immediately after the grommet has been inserted.
